Referring a Patient

At Honolulu Integrative PT we look forward to collaborating and contributing to the well-being of your patients!

To refer a patient, fax our Referral Form with the completed information to +1 (808) 791-2513.

Once we have received the form, we will contact the patient within a business day to schedule an appointment. After the initial evaluation, we will send via fax the evaluation and plan of care for your review. You will receive progress notes throughout the duration of care as needed.

Insurances Accepted: UHA, HMSA, HMAA PPO. Additionally, we accept cash payment and can provide patients with a Superbill upon request.
